For insurance purposes and due to the nature of the activities a consent form must be fully complete for each child prior to all parties and bespoke group events to enable your child to participate.

DO NOT COMPLETE IF YOU HAVE ALREADY COMPLETED A CONSENT FORM PREVIOUSLY - only one consent form is required unless any information has changed.
